How I ranked these programs. I look past websites, admissions scripts and amenity lists. The person arriving for treatment is often frightened, exhausted and not sure whom to trust. The best program is the one that can assess that person honestly, build a plan around the patient instead of a template, treat co-occurring conditions, involve family when appropriate, maintain a safe and respectful environment, and stay connected after discharge. Those priorities drive this ranking.

The Ohio Auditor of State issued an adverse Medicaid-compliance opinion concerning IBH for selected 2020-2022 claims and identified $43,493.79 in improper payments, plus $3,365.11 in calculated interest. IBH reported that it had changed its workflows and controls beginning in late 2022, but the auditor did not test that response. The auditor also issued a qualified opinion concerning Meridian HealthCare for selected 2020-2022 claims and identified $5,282.85 in improper payments, plus $971.54 in calculated interest; Meridian declined to submit an official response. Neither report is a published license suspension, and both organizations remain active with current independent accreditation. Still, these findings require a real scoring penalty. IBH moves from fifth to tenth, and Meridian’s score is reduced.
