The Harm Reduction Therapy Center (HRTC) delivers integrated mental health and substance use care for individuals whose drug use is tightly connected to their emotional histories, social realities, and daily survival. Instead of relying on oversimplified “just say no” approaches, HRTC meets people where they are, recognizing that meaningful change begins with trust, compassion, and an understanding of the complex circumstances that shape each person’s life.
Since 2018, HRTC has operated weekly mobile “clinics without walls” across four San Francisco neighborhoods. These street-based sites prioritize basic needs first such as food, safety, and connection. This approach creates a welcoming therapeutic space that naturally draws hundreds of community members seeking counseling, support, and practical assistance.
A large portion of HRTC’s clients are unhoused and often feel unwelcome or misunderstood in traditional service settings. To reduce barriers, all treatment environments are intentionally low-threshold. The organization’s main center, a thoughtfully designed warehouse space in SOMA, is complemented by more than a dozen community-based locations where harm reduction therapy is offered directly to those who need it most.
HRTC also provides specialized services for unhoused youth, many of whom have survived severe trauma and built their own street-based communities. In partnership with local agencies, HRTC continues to pilot innovative approaches that honor these young people’s resilience while offering the acceptance and stability they deserve.
