Project Transition offers community-centered, therapeutic programs designed to support individuals with serious mental illness and dual diagnoses on their journey to recovery. Known as the “Traditional Program,” these community-based initiatives provide members with structured psychiatric rehabilitation services in an apartment setting, fostering both independence and social integration. Located in Philadelphia and Reading, PA; Wilmington, NC; and Nashville and Knoxville, TN, Project Transition’s programs allow members to live in shared apartments, with other residents in the complex who are not part of the program, promoting community connection and reducing stigma.
Each program consists of approximately 30 members who receive daily support through life skills workshops, wellness programs, individual and group therapy sessions, and community meetings held at nearby off-site centers. Project Transition emphasizes evidence-based treatment, particularly Dialectical Behavior Therapy (DBT), along with family engagement and vocational support, all coordinated with members’ physical healthcare providers. Staff are available on-site daily from 7 a.m. to 9 p.m., with 24/7 access to skills coaching and crisis support, ensuring members have consistent access to help when needed.
Project Transition’s long-term recovery model centers around creating a “therapeutic community” where members work toward milestones in areas such as work readiness, community integration, and personal management, ultimately empowering them to build a meaningful life within their communities.
